    On the mechanism of trailing vortex wandering

    The mechanism of trailing vortex wandering has long been debated and is often attributed to either wind-tunnel effects or an instability. Using particle image velocimetry data obtained in the wake of a NACA0012 airfoil, we remove the effect of wandering from the measured velocity field and, through a triple decomposition, recover the coherent wandering motion. Based on this wandering motion, the most energetic structures are computed using the proper orthogonal decomposition (POD) and exhibit a helical mode with an azimuthal wavenumber of |m|=1 whose kinetic energy grows monotonically in the downstream direction. To investigate the nature of the vortex wandering, we perform a spatial stability analysis of a matched Batchelor vortex. The primary stability mode is found to be marginally stable and nearly identical in both size and structure to the leading POD mode. The strikingly similar structure, coupled with the measured energy growth, supports the proposition that the vortex wandering is the result of an instability. We conclude that the cause of the wandering is the non-zero radial velocity of the |m|=1 mode on the vortex centreline, which acts to transversely displace the trailing vortex, as observed in experiments. However, the marginal nature of the stability mode prevents a definitive conclusion regarding the specific type of instability

    Commentary on strategies for switching antipsychotics

    Both the new generation of antipsychotics and the more traditional antipsychotic drugs produce an important and meaningful improvement in patients with schizophrenia, but most patients are neither cured nor free of symptoms. As a consequence, it is common to switch from one drug to another in the hope of obtaining a better response. All antipsychotic drugs produce some side effects, so switching can also be a tolerance issue. There are reports in the literature on the tactics of switching: abrupt discontinuation, cross tapering, starting a patient on a new drug while continuing with the old drug until the new drug has reached a steady state, or some variation on these tactics. In this issue, Ganguli et al. have carried out a randomized switching study, the data from which indicates the tactics that might be optimal. We put this paper into context, provide a critique and describe indications for switching

    Does self-monitoring of blood glucose improve outcome in type 2 diabetes? The Fremantle Diabetes Study

    AIMS/HYPOTHESIS: To assess whether self-monitoring of blood glucose (SMBG) is an independent predictor of improved outcome in a community-based cohort of type 2 diabetic patients. MATERIALS AND METHODS: We used longitudinal data from (1) 1,280 type 2 diabetic participants in the observational Fremantle Diabetes Study (FDS) who reported SMBG and diabetes treatment status at study entry (1993–1996), and (2) a subset of 531 participants who attended six or more annual assessments (referred to as the 5-year cohort). Diabetes-related morbidity, cardiac death and all-cause mortality were ascertained at each assessment, supplemented by linkage to the Western Australian Data Linkage System. RESULTS: At baseline, 70.2% (898 out of 1,280) of type 2 patients used SMBG. During 12,491 patient-years of follow-up (mean 9.8 ± 3.5 years), 486 (38.0%) type 2 participants died (196 [15.3%] from cardiac causes). SMBG was significantly less prevalent in those who died during follow-up than in those who were still alive at the end of June 2006 (65.4 vs 73.0%, p = 0.005). In Cox proportional hazards modelling, after adjustment for confounding and explanatory variables, SMBG was not independently associated with all-cause mortality, but was associated with a 79% increased risk of cardiovascular mortality in patients not treated with insulin. For the 5-year cohort, time-dependent SMBG was independently associated with a 48% reduced risk of retinopathy. CONCLUSIONS/INTERPRETATION: SMBG was not independently associated with improved survival. Inconsistent findings relating to the association of SMBG with cardiac death and retinopathy may be due to confounding, incomplete covariate adjustment or chance

    Meta-Analysis of Prevalence and Risk Factors for Cognitive Decline and Improvement After Transcatheter Aortic Valve Implantation

    Changes to cognition, both decline and improvement, are commonly reported after transcatheter aortic valve implantation (TAVI). However, previous systematic reviews and meta-analyses have missed these subgroups by assessing whole-group-averages for cognitive outcomes. We sought to pool estimates to identify the prevalence of cognitive decline and improvement after TAVI, as well as associated factors for these outcomes. A systematic review identified 15 articles appropriate for meta-analysis. When robust cognitive change definitions were employed, the pooled prevalence of incident cognitive impairment up to 1-, 1 to 6-, and ≥6-months post-TAVI was 7%, 14%, and 12%, respectively. For cognitive improvement, the prevalence from 1 to 6 months and ≥6 months after TAVI was estimated to be 19% and 11%, respectively. Two factors were associated with these cognitive outcomes: (1) using a cerebral embolic protection device was associated with decreased prevalence of cognitive decline up to 1-week post-TAVI; (2) baseline cognitive impairment had a large association with post-TAVI cognitive improvement. In conclusion, cognitive decline and cognitive improvement are experienced by approximately 7% to 19% of patients after TAVI, respectively. Those with the lowest cognitive performance pre-TAVI appear to have the most to gain in terms of cognitive improvement post-TAVI. Identifying further predictive factors for cognitive decline and improvement post-TAVI will facilitate a personalized-medicine approach for cognitive care and prognosis

    Mining and analysis of audiology data to find significant factors associated with tinnitus masker

    Objectives: The objective of this research is to find the factors associated with tinnitus masker from the literature, and by using the large amount of audiology data available from a large NHS (National Health Services, UK) hearing aid clinic. The factors evaluated were hearing impairment, age, gender, hearing aid type, mould and clinical comments. Design: The research includes literature survey for factors associated with tinnitus masker, and performs the analysis of audiology data using statistical and data mining techniques. Setting: This research uses a large audiology data but it also faced the problem of limited data for tinnitus. Participants: It uses 1,316 records for tinnitus and other diagnoses, and 10,437 records of clinical comments from a hearing aid clinic. Primary and secondary outcome measures: The research is looking for variables associated with tinnitus masker, and in future, these variables can be combined into a single model to develop a decision support system to predict about tinnitus masker for a patient. Results: The results demonstrated that tinnitus maskers are more likely to be fit to individuals with milder forms of hearing loss, and the factors age, gender, type of hearing aid and mould were all found significantly associated with tinnitus masker. In particular, those patients having Age<=55 years were more likely to wear a tinnitus masker, as well as those with milder forms of hearing loss. ITE (in the ear) hearing aids were also found associated with tinnitus masker. A feedback on the results of association of mould with tinnitus masker from a professional audiologist of a large NHS (National Health Services, UK) was also taken to better understand them. The results were obtained with different accuracy for different techniques. For example, the chi-squared test results were obtained with 95% accuracy, for Support and Confidence only those results were retained which had more than 1% Support and 80% Confidence. Conclusions: The variables audiograms, age, gender, hearing aid type and mould were found associated with the choice of tinnitus masker in the literature and by using statistical and data mining techniques. The further work in this research would lead to the development of a decision support system for tinnitus masker with an explanation that how that decision was obtained

    Molecular dynamics simulation of nonlinear waves in granular media

    Discrete dynamic equations of spheres in granular chains have been developed so as to simulate the evolution of acoustic signals in these media. The model was built based on Hertzian laws as well as the dissipation effect within the system, and the contact dynamics involving both compression and separation between spheres was also modelled. A molecular dynamics simulation method using the Velocity Verlet algorithm was developed to solve the equations. The strongly nonlinear solitary wave impulses are predicted by the numerical calculations and match the experimental results well. The simulation system has been used as a design tool to determine the optimal chain structure in term of the bandwidth and frequencies which are required within the output impulses. The results exhibit great potential in biomedical applications

    The Dynamic Excitation of a Chain of Pre-Stressed Spheres for Biomedical Ultrasound Applications: Contact Mechanics Finite Element Analysis and Validation

    There has been recent interest in the transmission of acoustic signals along a chain of spheres to produce waveforms of relevance to biomedical ultrasound applications. Effects which arise as a result of Hertzian contact between adjacent spheres can potentially change the nature of the signal as it propagates down the chain. The possibility thus exists of generating signals with a different harmonic content to the signal input into one end of the chain. This transduction mechanism has the potential to be of use in both diagnostic and therapeutic ultrasound applications, and is the object of the study presented here. The nonlinear dynamics of granular chains can be treated using discrete mechanics models. However, in cases where the underlying assumptions of these models no longer hold, and where geometries are more complex, a more comprehensive numerical solution must be sought. Contact mechanics problems can efficiently be treated using the finite element method. The latter was used to investigate the dynamics of a pre-stressed chain of six, 1 mm diameter stainless steel spheres excited at one end using a tone burst displacement signal with a fundamental frequency of 73 kHz. The final sphere of the chain was assumed to be in contact with a cylindrical matching layer radiating into a half-space of fluid with the properties of water. After addition of the fluid loading, radiated acoustic pressures in the medium were predicted. Comparison with experimental results suggests that finite element analysis is a suitable tool for investigating the design and performance of contact mechanics based transducers. Nevertheless, a better handle on the model input parameters as well as an improved experimental protocol are required to fully validate the model

    The generation of impulses from narrow bandwidth signals using resonant spherical chains

    An ultrasonic horn operating at 73 kHz has been used to excite one end of a chain of steel spheres. The signal transmitted along the chain was measured at the far end using a laser vibrometer. Various chain lengths, ranging from 2-10 spheres, have been studied. It was found that a set of solitary wave impulses were generated when a high input amplitude and a minimal pre-compression force was used. Both harmonics and sub-harmonics of the input frequency could be observed. Theoretical models were developed, based on the relevant equations of motion, which modelled the end conditions properly, and their predictions confirmed the experimental observations. Solitary wave impulses were generated only when certain numbers of spheres were used, corresponding to the establishment of resonances in the form of nonlinear normal modes (NNMs). It was found that longer chains led to wider impulse bandwidths. Increased pre-compression tended to damp out this phenomenon, but gave a weakly non-linear state where the time delay of propagation along the chain changed with applied static force. It was thus established that a gated sinusoidal input could be transformed into a set of impulses, of interest in biomedical applications
